At the core of this process lies understanding and accurately tracking a woman's ovulation cycle. Ovulation tracking is an indispensable guide for couples trying to conceive naturally, as well as for those undergoing treatments like intrauterine insemination (IUI) or in vitro fertilization (IVF). As Prof. Dr. Mehmet Çınar, in this article, we will explain in detail how ovulation tracking is performed, the differences in natural and medicated cycles, and how we manage this process at our Ankara IVF Center.
Signs of Ovulation
The female body provides various signals during the ovulation period, indicating this important event. Recognizing these signs is the first step for couples planning a natural pregnancy. However, it's important to remember that these signs can vary from person to person and do not always provide definitive information.
- Rise in Basal Body Temperature (BBT): Immediately after ovulation, under the influence of the progesterone hormone, body temperature typically rises by 0.3-0.6°C and remains elevated until the next menstrual period. Regularly recording basal body temperature every morning upon waking, before getting out of bed, can provide clues about the time of ovulation.
- Changes in Cervical Mucus: As ovulation approaches, the amount of cervical discharge (cervical mucus) increases and becomes clear, stretchy, and similar in consistency to egg whites. This is known as "fertile mucus," which facilitates sperm's journey into the uterus. After ovulation, the mucus decreases, thickens, and becomes sticky.
- Mild Pelvic Pain (Mittelschmerz): Some women may experience mild, often one-sided, pelvic pain or cramping during or shortly before ovulation. This pain is associated with the growth of the follicle or the release of the egg.
- Ovulation Test Kits (LH Tests): Urine-based ovulation test kits available in pharmacies detect the sudden surge in luteinizing hormone (LH) levels (LH surge). An LH surge is one of the most reliable indicators that ovulation will generally occur within 24-36 hours. These tests are a practical method of ovulation tracking that can be easily used at home.
While these signs offer important clues during the ovulation tracking process, resorting to medical methods is often necessary to obtain the most accurate and definitive information, especially if pregnancy has not occurred for some time.
Follicle Tracking with Ultrasound
The most reliable and scientific method for ovulation tracking is monitoring follicle development with gynecological ultrasonography. This method is vital, especially for women with irregular menstrual cycles or couples undergoing assisted reproductive treatments. At our Ankara IVF Center, as Prof. Dr. Mehmet Çınar and his team, we meticulously perform follicle tracking.
On specific days of the menstrual cycle (typically starting on day 2 or 3 of menstruation), the size and number of follicles in the ovaries are assessed using transvaginal ultrasonography. Follicles are fluid-filled sacs that contain the egg cell. As ovulation approaches, usually one, or rarely several, follicles become dominant and continue to grow. This dominant follicle is considered ready for ovulation when it reaches an average diameter of 18-22 mm.
During ultrasound monitoring, not only the size of the follicles but also the thickness and structure of the uterine lining (endometrium) are evaluated. The endometrium reaching a sufficient thickness and having a triple-line (trilaminar) appearance indicates the presence of a suitable environment for embryo implantation. This is particularly important in IUI (Intrauterine Insemination) and IVF (In Vitro Fertilization) treatments.
In addition to ultrasonography tracking, hormone levels (estrogen (E2), luteinizing hormone (LH), and progesterone (P)) can be measured with blood tests when necessary to determine the time of ovulation more precisely. Estrogen levels increase as the follicle grows, a sudden rise in LH levels indicates that ovulation is triggered, and an increase in progesterone levels confirms that ovulation has occurred.
Through this comprehensive monitoring, the optimal timeframe for the egg's release is determined. For natural conception attempts, couples are advised on the timing of intercourse. In IUI treatment, the collection and placement of the semen sample into the uterus are planned based on this data, and in IVF treatment, the egg retrieval (OPU) procedure is scheduled accordingly.
When is Medicated Tracking Necessary?
In some cases, it may be determined that women do not ovulate regularly through natural means or that egg quality is insufficient. In such situations, medicated ovulation tracking and stimulation are pursued to increase the chances of pregnancy. Medicated tracking may be recommended particularly for the following conditions:
- Polycystic Ovary Syndrome (PCOS): In women with irregular or no ovulation, medication is administered to stimulate the ovaries and induce ovulation.
- Unexplained Infertility: In couples where pregnancy has not been achieved despite all tests being normal, medicated tracking may be attempted to enhance ovulation potential.
- Low Ovarian Reserve: In women with diminished ovarian reserve, medication can be provided to ensure more efficient use of the remaining eggs.
- Assisted Reproductive Technologies: In treatments such as IUI (Intrauterine Insemination) and IVF (In Vitro Fertilization), ovarian stimulation medications are used to obtain multiple eggs and control the timing of ovulation. The number and quality of eggs to be retrieved are of great importance, especially in advanced techniques like Microinjection (ICSI).
During medicated tracking, oral medications or injections specifically determined for the woman's condition are used. These medications aim to stimulate the ovaries, encouraging the growth of multiple follicles. The dosage and duration of treatment are individually planned based on the patient's age, ovarian reserve, previous treatment responses, and overall health status. Medications should never be used without medical supervision, and dosage adjustments should not be made independently.
The medicated tracking process requires more intensive monitoring compared to natural cycle tracking. Follicle development, endometrial thickness, and hormone levels are closely monitored through regular ultrasonographic examinations and blood tests. Once the follicles reach the desired size, an ovulation-triggering injection (trigger shot) is administered, and egg retrieval or IUI is planned approximately 34-36 hours after this injection.

The Importance of Timing
In the journey to achieve pregnancy, accurate timing determined by ovulation tracking is one of the keys to success. Whether attempting natural conception or utilizing assisted reproductive technologies, the importance of timing is undeniable in both scenarios.
- For Natural Conception Attempts: After being released from the ovary, an egg is capable of fertilization for approximately 12-24 hours. Sperm, on the other hand, can remain viable in the female body for 3-5 days. Therefore, engaging in intercourse 2-3 days before ovulation and on the day of ovulation maximizes the chances of pregnancy. This period is known as the "fertile window." Ovulation tracking allows this window to be accurately determined, and couples can adjust the timing of intercourse accordingly.
- In IUI (Intrauterine Insemination) Treatment: In IUI treatment, the goal is to inject concentrated and motile sperm, prepared in a laboratory setting, into the uterus at the time of ovulation. The timing of this procedure is set a specific period after the ovulation-triggering injection, following precise determination of ovulation through ultrasound and hormone tests. Correct timing increases the likelihood of sperm meeting the egg.
- In IVF (In Vitro Fertilization) Treatment: In the IVF process, the egg retrieval (OPU) procedure must be performed at the most opportune time, when follicles have reached optimal size and eggs have matured. This procedure
