What is Varicocele and its Relationship with Infertility
Varicocele is a condition where the veins surrounding the testicles become enlarged and swollen, similar to varicose veins in the legs. It is one of the most common correctable causes of male infertility. Varicocele negatively affects sperm production and quality by slowing blood flow to the testicles and increasing scrotal temperature. The increased temperature can lead to sperm DNA damage, reduced sperm motility, and the formation of abnormally shaped sperm.
Varicocele often presents no noticeable symptoms and is usually diagnosed during a routine physical examination or infertility investigations. Some men may experience complaints such as pain, a feeling of heaviness, or a difference in testicular size.
Diagnosis and Treatment Approaches
Varicocele is typically diagnosed through a physical examination and scrotal ultrasonography. Ultrasonography provides detailed visualization of enlarged veins and blood flow. Treatment usually involves surgical intervention to ligate or block the enlarged veins (varicocelectomy). The goal of this operation is to normalize blood flow to the testicles and improve sperm production by reducing scrotal temperature. Not every varicocele case requires surgery; the decision for treatment is made considering factors such as the degree of varicocele, sperm parameters, and the couple's desire to have children.

What is Intracytoplasmic Sperm Injection (ICSI)?
ICSI is a specialized form of In Vitro Fertilization (IVF) treatment, particularly used in cases of severe male infertility. In this method, a single healthy sperm cell, selected under a microscope in the laboratory, is directly injected into an egg cell using a special microneedle. This allows fertilization to occur even if the sperm is unable to penetrate the egg on its own. ICSI offers high success rates for issues such as low sperm count (oligospermia), poor sperm motility (asthenospermia), or abnormal sperm morphology (teratospermia). Surgical Sperm Retrieval Methods (TESE/Micro-TESE)
In some men, no sperm may be present in the ejaculate (azoospermia). This condition can be due to a problem with sperm production or a blockage in the sperm ducts. In such cases, it is possible to surgically retrieve sperm directly from the testicles. These methods include:
- TESA (Testicular Sperm Aspiration): Sperm is aspirated from the testicle using a needle.
- TESE (Testicular Sperm Extraction): A small piece of testicular tissue is taken to search for sperm.
- Micro-TESE (Microdissection TESE): Under an operating microscope, areas of the testicle with high sperm production potential are precisely identified and sperm is searched for.
