How is Grading Performed?
Embryo grading is the process by which our embryologists carefully examine embryos under a microscope to determine their developmental potential. This process relies entirely on visual criteria and assesses characteristics such as the embryo's cell number, cell symmetry, fragmentation (cell breakdown) rate, and the quality of the inner cell mass (ICM) and trophectoderm (TE). Based on these observations, our embryologists assign a specific grade or score to each embryo.
Grading of Cleavage Stage (Day 2-3) Embryos
- Cell Number and Division Rate: Ideally, 2-4 cells are expected on Day 2, and 6-8 cells on Day 3. Regular and timely cell division indicates good developmental potential.
- Cell Symmetry: The cells (blastomeres) forming the embryo are preferred to be similar in size and symmetrical.
- Fragmentation Rate: Fragmentation, also known as cell breakdown, refers to small, anucleated cell fragments that can negatively affect embryo development. The lower the fragmentation rate, the better the embryo quality is considered. Generally, fragmentation rates below 10% are evaluated as good.
Grading of Blastocyst Stage (Day 5-6) Embryos
When an embryo develops to Day 5 or 6 in the laboratory, it reaches the blastocyst stage. Transfer at this stage allows for higher accuracy in embryo selection and can increase pregnancy rates. Blastocyst grading is generally based on three main criteria:
- Degree of Expansion: The volume of the blastocyst and its degree of hatching from the outer shell (zona pellucida) are assessed. Fully expanded or beginning-to-hatch blastocysts are considered better.
- Inner Cell Mass (ICM) Quality: This is the group of cells that will form the baby. A dense, well-organized ICM indicates a high-quality embryo.
- Trophectoderm (TE) Quality: This is the cell layer that will form the placenta. Regular, numerous, and well-organized trophectoderm cells indicate the embryo's potential to implant in the uterus.
These grading systems provide our embryologists with valuable information on which embryo is most suitable for transfer. However, it should be remembered that this is a visual assessment and does not directly provide information about the embryo's genetic structure. At this point, advanced tests such as Preimplantation Genetic Testing (PGT) can be employed when deemed necessary.
Embryo Development Day by Day
The IVF process begins with egg retrieval and fertilization, and each developmental stage of the embryo in the laboratory is meticulously monitored. Here's the day-by-day development of an embryo:
- Day 0 (Fertilization): After egg retrieval, fertilization with sperm is performed. This is usually done using the intracytoplasmic sperm injection (ICSI) method. Successful fertilization is checked 16-18 hours later by the presence of two pronuclei.
- Day 1 (Zygote): The fertilized egg is now called a zygote. Cell division has not yet begun at this stage.
- Day 2 (2-4 Cell Embryo): The zygote begins to divide and transforms into an embryo consisting of 2-4 blastomeres. Cell symmetry and fragmentation rate are assessed at this stage.
- Day 3 (6-8 Cell Embryo): The embryo reaches 6-8 cells. This is one of the most frequently preferred stages for cleavage-stage embryo transfer. Cell number, symmetry, and fragmentation remain critically important at this stage.
- Day 4 (Morula): The embryo transforms into a mulberry-like structure with an increasing number of cells and is called a morula. The cells begin to compact tightly, making it difficult to distinguish them individually from the outside.
- Day 5-6 (Blastocyst): The morula forms a fluid-filled cavity (blastocoel) and transitions to the blastocyst stage. At this stage, the inner cell mass (ICM) and trophectoderm (TE) layers become distinct. Blastocyst transfer offers a more precise evaluation opportunity for embryo selection and is generally associated with higher pregnancy rates.
