Dreaming of parenthood is a natural wish at every stage of life. Today, many women postpone motherhood for a variety of reasons — a career, education or personal goals. The biological clock, however, is an important factor in fertility, particularly for women aged 40 and over. This leads many couples to ask questions such as "Is IVF treatment possible over the age of 40?" or "What are the chances of success with IVF at an advanced age?" Drawing on more than 30 years of experience at our Ankara IVF Center, Prof. Dr. Kubilay Vicdan would like to set out realistic expectations for this particular stage of life and the roadmap to follow. Remember: with modern medicine and personalised approaches, there is always hope.
The Effect of Age on Fertility
Female fertility naturally declines with age. This decline accelerates especially after the age of 35 and becomes more marked after 40. The main reasons are:
- Ovarian reserve and egg quality: Women are born with a set ovarian reserve, and this reserve diminishes with age. As the number of eggs falls, the quality of the remaining eggs also declines. This drop in quality increases the risk that the genetic material (the chromosomes) in the eggs will differ from normal. That can adversely affect the healthy development of the embryo, reducing the chance of pregnancy while raising the risk of miscarriage.
- Hormonal changes: With age, the way the ovaries respond to hormones changes. This can lead to irregular menstrual cycles, ovulation problems and difficulties in the lining of the uterus becoming receptive to the embryo.
- Other health problems: With advancing age, additional gynaecological conditions that can affect fertility — such as fibroids, endometriosis or blocked tubes — become more frequent.
In the light of these factors, achieving a pregnancy naturally can become more difficult for women over 40. Assisted reproductive techniques such as IVF therefore become an important option for couples in this age group. Although there is no legal upper age limit for IVF, an age limit does mark the point at which a woman's biological potential to conceive with her own eggs decreases. Even so, modern medicine can offer tailored solutions despite these challenges.
The Treatment Plan Over the Age of 40
To achieve successful results with IVF over the age of 40, a personalised and detailed treatment plan is essential. At the Ankara IVF Center we assess every patient's situation individually. The process generally involves the following steps:
Comprehensive Assessment
Before treatment begins, the general health of the woman and her partner, their fertility potential and any previous treatment history are examined in detail.
- Ovarian reserve tests: Tests such as AMH (anti-Müllerian hormone), FSH (follicle-stimulating hormone) and the antral follicle count provide important information about the ovarian reserve. These tests guide us on how the ovaries are likely to respond to stimulation.
- General health check: We assess whether chronic conditions such as diabetes, thyroid disease or hypertension are present and whether they are under control. Where necessary, support is sought from the relevant specialties.
- Assessment of the uterus: The internal structure of the uterus and the condition of the tubes are examined using methods such as ultrasound, hysteroscopy or laparoscopy. Conditions that could prevent a pregnancy, such as fibroids, polyps or adhesions, are identified and treated.
Personalised Stimulation Protocols
In women in the older age group, the way the ovaries are stimulated is of great importance. The goal is to obtain good-quality eggs, even if they are few in number. To that end:
- Modified or mild stimulation: In some cases gentle stimulation of the ovaries with lower drug doses may be preferred. This aims to improve the quality of the eggs obtained while avoiding overstimulation of the ovaries.
- Antagonist protocols: Antagonist protocols, which are generally more flexible and carry a lower potential for side effects, may be preferred.
Because every woman responds differently, the ovarian response to the medication is closely monitored throughout treatment and doses are adjusted accordingly.
Advanced Laboratory Techniques
Modern laboratory techniques are used at the egg collection and fertilisation stages. Microinjection (ICSI) in particular is widely preferred in order to increase the chance of fertilisation whatever the quality of the egg.
Approaches That Improve Success
A multidisciplinary approach and the use of advanced technology are critically important in improving success rates in IVF over the age of 40.
Preimplantation Genetic Testing (PGT)
The decline in egg quality at an older age increases the risk of chromosomal abnormalities in embryos. This makes it harder to achieve a pregnancy and raises the risk of miscarriage. Preimplantation Genetic Testing (PGT) allows embryos to be examined genetically before they are transferred into the uterus. By making it possible to select and transfer embryos found to be chromosomally normal, PGT can increase the chance of pregnancy, reduce the risk of miscarriage and improve the likelihood of having a healthy baby. This is a particularly valuable approach for IVF patients over the age of 40.
Embryo Quality and Transfer
Following embryo development closely in the laboratory and selecting the highest-quality, fully developed embryos (blastocysts) for transfer increases the chance of success. The embryo transfer is carried out carefully and gently, at the moment when the lining of the uterus is most receptive to implantation.
Lifestyle Changes and Supportive Treatments
It is very important that both partners pay attention to their general health during treatment:
- Healthy nutrition: A balanced diet rich in antioxidants can support egg and sperm quality.
- Regular exercise: Regular exercise that is not taken to excess reduces stress and improves general health.
- Stress management: The IVF process can be demanding both physically and emotionally. Managing stress through methods such as yoga, meditation or counselling can have a positive effect on the course of treatment.
- Avoiding smoking and alcohol: These habits have been scientifically proven to have a negative effect on fertility.
In some cases options such as egg freezing or embryo freezing may also be considered. Egg freezing in particular can be an important strategy for preserving fertility potential at a later age, especially with future pregnancy plans in mind.
Managing Realistic Expectations
Although IVF over the age of 40 is a promising path, having realistic expectations during the process is very important. At the Ankara IVF Center we always give our patients transparent information based on scientific evidence.
Success Rates and Individual Differences
Success rates in IVF treatment at an older age may be lower than in younger age groups. The main reason for this is the decrease in ovarian reserve and egg quality. This does not mean, however, that success is impossible. Every individual's situation is different, and the chance of success depends on many factors, including ovarian reserve, general health, and any previous pregnancy or treatment history. Some women aged 40 and over are able to conceive with their own eggs, while for others the process may be longer and more demanding.
Emotional and Psychological Support
The IVF process can be emotionally exhausting for couples, especially at an older age. The uncertainty that treatment brings, hormonal changes and repeated attempts can lead to stress and anxiety. For this reason, receiving psychological support during treatment is important in protecting emotional wellbeing and managing the process in a healthier way. Our centre has specialists who can support our couples on this challenging journey.
Patience and Persistence
IVF treatment over the age of 40 sometimes requires more than one attempt. Each attempt calls for both a physical and an emotional investment. Throughout this process it is vital to be patient, to keep your determination and to remain in open communication with your doctor. The data obtained from each treatment cycle is used to plan the next step more effectively. The IVF process is a personalised journey that advances step by step.
It should not be forgotten that our aim is not simply to achieve a pregnancy, but also to bring about a healthy pregnancy and a healthy baby. To reach that goal, being treated at a centre equipped with an experienced team and modern technology will increase your chances.
