- Hysteroscopy: This is an endoscopic method that allows direct visualization of the cervical canal and uterine cavity. It can be used to confirm the diagnosis and treat suspicious anatomical abnormalities, polyps, or adhesions in the same session. Solution with Intrauterine Insemination (IUI)
In infertility caused by cervical factor, the primary goal is to enable sperm to overcome cervical barriers and reach the uterus. In this regard, one of the first-line treatment options is usually intrauterine insemination (IUI).
IUI treatment is based on the principle of directly placing specially prepared and concentrated sperm into the uterus. This method achieves the following:
- Cervical Barrier Overcome: The necessity for sperm to overcome the cervical mucus barrier or pass through anatomical narrowing is eliminated.
- Increased Sperm Concentration: The prepared sperm sample contains a higher concentration of motile and healthy sperm, which increases their chance of reaching the fallopian tubes.
- Optimized Timing: When performed in conjunction with ovulation tracking, it ensures that sperm are present in the uterus at the most opportune time for egg fertilization.
IUI treatment can be an effective option, especially in cases of mild to moderate cervical factor infertility, mild male factor infertility, and unexplained infertility. The treatment process typically involves mild ovarian stimulation, monitoring of egg development, and preparing and introducing the sperm sample into the uterus at the appropriate time. The success rate of IUI treatment varies depending on many factors, such as the woman's age, duration of infertility, presence of additional infertility factors, and sperm quality. If pregnancy is not achieved after several attempts, or if other causes of infertility are present, more advanced treatment options are considered.
Situations Requiring IVF Treatment
Although IUI is an effective first-line treatment for infertility caused by cervical factor, in some cases or when IUI attempts are unsuccessful, In Vitro Fertilization (IVF) treatment may be necessary. IVF is one of the most comprehensive infertility treatments and generally has the highest success rates.
IVF treatment can be considered a priority when other causes of infertility are present in addition to the cervical factor, or when pregnancy has not been achieved with IUI:
- IUI Failure: If pregnancy is not achieved despite 3-4 cycles of IUI attempts, IVF treatment may be recommended.
- Severe Cervical Factor: In situations such as severe anatomical abnormalities of the cervix (severe stenosis) or highly impaired mucus quality, IUI may not be sufficient. IVF completely bypasses this barrier.
- Additional Infertility Factors: If the woman has additional conditions such as advanced age, low ovarian reserve, blocked or damaged fallopian tubes (tubal factor), endometriosis, or if the male has severe sperm problems (severe male factor), IVF treatment becomes a more suitable option.
