TESA / TESE / Micro-TESE
Surgical sperm retrieval in azoospermia, combined with ICSI on the same day.
Azoospermia — no sperm in the ejaculate — affects about 1% of men, but it does not mean fatherhood is impossible. Sperm production often continues in small focal areas of the testis, and these sperm can be reached surgically.
Methods
- TESA: needle aspiration — the least invasive method
- TESE: open biopsy taking a tissue sample
- Micro-TESE: selecting sperm-producing tubules under an operating microscope — minimal tissue damage, highest chance of finding sperm (30-60%)
The procedure is performed by our urology specialist under anesthesia, with the embryology team examining the tissue simultaneously in the operating room. When sperm are found, they are injected into the partner’s eggs by ICSI on the same day; remaining sperm can be frozen.
Correctable causes such as varicocele are evaluated first; hormonal support can increase the chance of finding sperm.
