Hysteroscopy is an important diagnostic and therapeutic tool in women's health. It is frequently utilized, especially in infertility treatment, recurrent pregnancy losses, or IVF failures, to detect and correct intrauterine problems. How is Hysteroscopy Performed?
Hysteroscopy is a procedure where a thin, lighted telescope (hysteroscope) is inserted through the cervix to directly visualize the uterine cavity (endometrial cavity) and the cervical canal. This procedure is typically scheduled within the first week after menstrual bleeding has ceased, when the uterine lining (endometrium) is thinnest. During this period, the inside of the uterus can be seen more clearly, and potential pathologies are easier to detect.
The procedure can generally be performed under light anesthesia (sedation) or local anesthesia, preventing the patient from feeling any pain or discomfort. The hysteroscope is gently advanced through the cervix until it reaches the uterine cavity. During this, a special fluid (usually saline solution) or gas (carbon dioxide) is used to expand the uterus and provide a clearer view. This separates the uterine walls, allowing for a detailed examination.
The camera at the tip of the hysteroscope transmits images from inside the uterus to a high-resolution monitor. This allows the doctor to evaluate the uterine lining, the shape of the uterine cavity, the areas where the fallopian tubes join the uterus (ostia), and any potential abnormalities (such as polyps, fibroids, adhesions, or septum) in real-time. The procedure duration varies depending on whether it is diagnostic or operative, but generally lasts between 15-45 minutes.
Diagnostic and Operative Hysteroscopy
Hysteroscopy, according to its purpose, is divided into two main categories:
Diagnostic Hysteroscopy
As its name suggests, the primary purpose of diagnostic hysteroscopy is to diagnose potential problems within the uterus. This type of intrauterine evaluation is typically performed in the following situations:
- To evaluate intrauterine factors in cases of unexplained infertility.
- To investigate the cause of recurrent pregnancy losses.
- To determine the cause of abnormal uterine bleeding (non-menstrual bleeding, excessive bleeding).
- To confirm intrauterine lesions (polyps, fibroids, adhesions) suspected in other imaging methods like ultrasound or HSG (hysterosalpingography).
- To thoroughly examine the uterine cavity and ensure a suitable environment for pregnancy before IVF treatment.
- To confirm the presence of intrauterine adhesions (Asherman's syndrome).
During diagnostic hysteroscopy, the doctor can directly observe any abnormalities inside the uterus. If necessary, biopsies can be taken from suspicious tissues and sent for pathological examination. This is vitally important, especially in cases where intrauterine cancer is suspected.
Operative Hysteroscopy
Operative hysteroscopy is performed to simultaneously treat intrauterine pathologies detected during a diagnostic procedure or known beforehand. In this case, a wider hysteroscope and special surgical instruments are used. Conditions that can be treated with operative hysteroscopy include:
- Removal of intrauterine polyps (polypectomy): These are benign growths originating from the uterine lining that can hinder pregnancy or cause bleeding.
- Removal of small submucosal fibroids (myomectomy): Fibroids originating from the muscular layer of the uterus but growing into the uterine cavity can make embryo implantation difficult.
- Lysis of intrauterine adhesions (adhesiolysis): Adhesions that can form after previous surgeries, infections, or curettage can narrow the uterine cavity, preventing pregnancy or causing recurrent miscarriages.
- Correction of a uterine septum (septoplasty): This is a congenital uterine anomaly, a wall dividing the uterus into two, which can lead to recurrent pregnancy losses or premature birth.
- Removal of lost intrauterine devices (IUDs).
Operative hysteroscopy is significantly less invasive than open surgery, offers a shorter recovery period, and generally allows for same-day discharge from the hospital. This enables our patients to return to their daily lives more quickly.
Who is Hysteroscopy Recommended for Before IVF?
The success of in vitro fertilization (IVF) treatment is not limited solely to obtaining high-quality embryos; the uterine environment where the embryo will implant must also be healthy and conducive. Therefore, intrauterine evaluation performed before the IVF process or after unsuccessful attempts is of great importance. Hysteroscopy before IVF is particularly recommended for patients with certain risk factors or those who have experienced previous failures.
